All new students, as well as any student returning after taking a semester off (spring or fall), are required to complete an Academic Accommodation Request Form and submit supporting documentation through the MyWellness portal.
Step 1: Log into the MyWellness portal, select the "Request Accommodations" tab, and complete the Academic Accommodation Request Form.
Step 2: After each initial Academic Accommodation Request Form has been reviewed the student will receive a message from Disability Services in their MyWellness portal with instructions to schedule an Intake Appointment.
Students are encouraged to contact Disability Services if they have not received any
correspondence via the myWellness portal or by phone after three weeks from the initial
submission of the request.
Step 3: During an Intake Appointment the student engages in an interactive process with a disability counselor regarding the Academic Accommodation Request Form, supporting documentation, barriers experienced in the academic setting, requested accommodations, completion of necessary paperwork, policies/procedures, accommodation letters, available support services, and any other information that is relevant to the student. Accommodations become active once the student delivers their Academic Accommodation Letter to each instructor.
Accommodations will automatically renew each consecutive semester a student is enrolled for courses. However, any student who takes the fall or spring semester off is required to submit a new Academic Accommodation Request Form through the myWellness portal to reactivate accommodations.
