Required Documents

To finalize an EOP application, students must submit verification documents to prove both academic potential and financial eligibility. All documents (if applicable) must be uploaded directly to the college's applicant portal or the SUNY Application Services Center.

  • Income Verification - Social Security Administration
  • Form 4506-T
  • Child Support
  • EOP Recommendation Form (optional)

All Fall 2026 applicants must confirm taxable income meets eligibility guidelines and submit proof of non-taxable income from the 2024 calendar year to determine financial eligibility for EOP. 

Documents needed for the EOP application

Taxable Income Sources: Federal Tax Return 1040, or 1040A, or 1040EZ along with W-2s and schedules; and NY State Tax Return IT 201.

Non-Taxable Sources: Social Security, Social Services, Child Support, Veteran's Benefits, or Workers Compensation.
